In order to save time when coming in for your appointment or when dropping off your information, please fill out the Client Organizer we sent you via e-mail prior to your appointment. If you have not received a Client Organizer from us or have misplaced yours, you can print and fill out the appropriate Client Organizer below. The Client Organizer ensures that you have not missed any important information and will come to the office prepared. Also, you will find the Engagement Letter and Consent Form that must be signed and returned to us with your information.
